Helping Families Make Clear Decisions
We want you to make comfortable, educated choices about the care plan for your loved ones. One of Lakeview's registered nurses will serve as your case manager, visiting your hospital room to answer questions and evaluate your needs. This involves a chart review and discussion of your past medical history. Lakeview's social workers initiate all home care referrals as well as provide support and counseling to both patient and family. This promotes a smooth transition from Lakeview to the home.
While at Lakeview our residents have the opportunity to be involved in a variety of social activities and outings that promote community, and enhance their social well being. It is these positive experiences that add to their quality of life and raising their self-esteem. Lakeview staff work with patients and families to develop individualization and plan of care designed to help them reach their full potential.
Professional counseling is available to help patients and residents in dealing with emotional needs on day adjustment issues.
Physical Therapy is provided on a daily basis.
Family members are encouraged to take an active role in plan of care. We have care plan conferences held upon admission and as needed to meet the individual needs of each patient.
Monthly support group meetings are available on premises for caregivers that allow opportunity to verbalize and provide comfort and reassurance among a group of peers.
Family council meetings are held on a quantity basis that features guest speakers with expertise in many areas related to again and estate planning.
Evening hours are available to meet with social worker staff on a weekly basis
