Planning a Program


Following admission, our interdisciplinary team* develops a plan of care to map out the most efficient course of treatment. During this phase, we carefully monitor each patient's condition to ensure that you receive all the care necessary to get well. The case manager proactively provides written or telephone progress reports to keep your insurance company's case manager informed of your progress.

Rehabilitation


Patients are evaluated by our staff physiatrist*, and a plan of care is prescribed. The physiatrist attends weekly meetings with the interdisciplinary team to monitor your progress and adjust the care plan accordingly. Throughout the process, we work closely with your family doctor or can refer one if needed.

*SUBACUTE CARE: is a comprehensive, cost-effective and outcome-oriented approach to care for patients requiring short-term, complex physical or medical rehabilitation from a physician-directed interdisciplinary team. Subacute programs typically are utilized as an alternative to prolonged hospitalization or as an inpatient alternative to an acute care hospital stay.
