Helping Families Make Clear Decisions

 

Social work staff works with each patient and their families to develop an individualized plan of care that will help them reach their full potential. Family members are encouraged to take an active role in the plan of care with care plan conferences offered upon admission and as needed, to meet the individual
needs of each patient.

Social Workers provide assessment of patient’s psychosocial needs & encourage activities that promote self-determination and psychosocial well-being. Counseling services are available to assist patients in dealing with any emotional and adjustment issues.

Monthly support group meetings are available on premises for caregivers, to allow verbalization and to provide comfort and reassurance amongst a group of peers.

Family council meetings are held on a quarterly basis that feature guest speakers with expertise in many areas related to aging and long-term planning.

When a patient's goals have been met, our primary focus is on patient safety and satisfying the patient’s discharge needs. Social work staff assist patients and their families in applying for community entitlements and available programs to ease the transition in discharge back to home.

*SUBACUTE CARE: is a comprehensive, cost-effective and outcome-oriented approach to care for patients requiring short-term, complex physical or medical rehabilitation from a physician-directed interdisciplinary team. Subacute programs typically are utilized as an alternative to prolonged hospitalization or as an inpatient alternative to an acute care hospital stay.
